Most pulmonary emboli are caused by blood clots that form in the deep veins of the legs or pelvis (deep vein thrombosis). Less common causes include fat embolism after major fractures, air embolism, amniotic fluid embolism during childbirth, and, rarely, tumor emboli.

Diagnosis typically involves a medical history, physical examination, D-dimer blood test, CT pulmonary angiography (CTPA), ventilation-perfusion (V/Q) scan when appropriate, chest X-ray, electrocardiogram (ECG), echocardiogram, pulse oximetry, arterial blood gas (ABG), and ultrasound of the legs to detect deep vein thrombosis.

Treatment usually includes anticoagulant (blood-thinning) medications to prevent new clots from forming. Severe cases may require thrombolytic (clot-dissolving) therapy, catheter-directed treatment, or surgical removal of the clot. Oxygen therapy and supportive hospital care are provided when needed.

People at increased risk include those with a history of deep vein thrombosis or pulmonary embolism, recent surgery or hospitalization, cancer, inherited clotting disorders, obesity, pregnancy, prolonged immobility, smoking, heart disease, and those taking estrogen-containing medications such as some birth control pills or hormone replacement therapy.
