Sarcoidosis is an inflammatory disease in which small clusters of immune cells, called granulomas, form in different organs. The lungs and lymph nodes are most commonly affected, but the disease can also involve the eyes, skin, heart, liver, nervous system, and other organs.

Common symptoms include a persistent dry cough, shortness of breath, chest pain, fatigue, fever, night sweats, swollen lymph nodes, skin rashes, and eye problems such as redness or blurred vision. Some people have no symptoms and are diagnosed during routine imaging tests.

The exact cause of sarcoidosis is unknown. It is believed to result from an abnormal immune response to environmental or infectious triggers in people with a genetic predisposition. Sarcoidosis is not contagious and cannot be spread from person to person.

Diagnosis usually includes a medical history, physical examination, blood tests, chest X-ray, high-resolution CT (HRCT) scan, pulmonary function tests (PFTs), and often a biopsy. A biopsy showing non-caseating granulomas, after excluding other causes such as tuberculosis and fungal infections, supports the diagnosis.

Many people recover without treatment, while others require medication to control inflammation. There is no guaranteed cure, but early diagnosis and appropriate treatment can relieve symptoms, protect affected organs, and reduce the risk of long-term complications.

Treatment depends on the severity of the disease and the organs involved. Options may include observation for mild cases, corticosteroids (such as prednisone), immunosuppressive medications, biologic therapies for refractory disease, oxygen therapy, pulmonary rehabilitation, and, in rare advanced cases, lung transplantation.
